Jason Jensen, Director of Operations
Mr. Jensen has been in administration in hospitals, transitional care units (TCUs), and skilled nursing facilities (SNF) since 2006. He has operated facilities in states across the West. He has been a board member on Idaho’s Healthcare Association for assisted living and skilled nursing facilities. His abilities to guide troubled facilities back into compliance and lead facilities to qualify for and receive the State of Idaho’s L. Jean Schoonover Quality Care Award are among his many strengths as an administrator. He was the recipient of Idaho Region Star Garnet ‘Administrator of the Year’ Award in 2008. He provides consulting to Assisted Living facilities, Home Health and Hospice Companies, and Skilled Nursing Facilities.
Chukk Nielson, RN, BSN, Director of Clinical Operations and Compliance
Mr. Nielson has had the opportunity to be a nurse in the healthcare field for the past 7 years. Mr. Nielson graduated summa cum laude from Boise State University, and is currently seeking his doctoral degree in nursing at Duke University. He has worked ambulatory care, mental health, home health, hospice, esthetics, and skilled nursing. Mr. Nielson was the Director of Nursing for our skilled nursing facility in Boise, Idaho, and has consulted for our skilled nursing facilities. He is currently the Director of Clinical Operations and Compliance for our company.
Austin Loders, Recruiting Coordinator
Mr. Loders joins Promontory Healthcare Companies as a recent graduate of Concordia University in Portland, Oregon. He graduated in 2016 with a Bachelor of Science degree in Homeland Security and Emergency Management, along with a minor in Business Administration. While attending Concordia, he was also a member of the men’s baseball team and a member of the 2016 Academic All-GNAC team. He is currently the Recruiting Coordinator for our company, and hopes to connect patients with quality healthcare professionals to ensure the road to recovery is paved with comfort and dignity.
Rich Cartney, Old Mill Rehabilitation
Mr. Cartney has been in healthcare management for the past 30 years, the past 20 years as administrator of long term care facilities. His experience includes hospitals, clinics, HMO, skilled nursing, assisted living, senior housing and low income housing. In 2014 he was selected as the AANAC Administrator of the Year for the US, and has received the J. Schoonover Award for nursing home quality in 2012 and 2013. In 2010, his facility was recognized for being in the top 10% nationwide for staff, resident and family satisfaction surveys. He has been active in healthcare associations in Idaho, New Mexico and South Dakota. He is a veteran of the Air Force, serving in Desert Storm in Saudi Arabia.
Tyler Fackrell, Promontory Point Rehabilitation
Tyler Fackrell, originally from Idaho Falls, ID joined us in February of 2016. He was administrator of two other facilities, one at 154 beds and the other at 97 beds in Tennessee and Virginia respectively. He is extremely excited to be back home and work with such a wonderful group of nursing staff and therapists. His focus is to keep customer care of utmost importance and maintaining Promontory Point as the number one place to receive rehab in the region.
Tyler received his bachelor’s degree from Brigham Young University-Idaho and received a Master’s degree in Health Sector Management from W.P. Carey School of Business at Arizona State.
Tyler enjoys motorbiking in the mountains and being with his family. He is father to 3 beautiful children, ages 8, 5, and 3, and husband to a wonderful wife of 10 years, Jenny Fackrell. He appreciates her support in all of his endeavors. Tyler plans to be with Promontory Point for many years.
Chuck Williams, Riverview Rehabilitation
Chuck Williams has been in Long Term Care in one capacity or another since 1975. It was while working as a housekeeper in a nursing home in Texas in 1975 that he decided to make LTC Administration his career. After college and working in nursing homes in La Grande Oregon, he was first licensed in Oregon in 1979. He has since run nursing homes in Oregon, California, Colorado and Idaho. He has led several facilities to receive prestigious care and operations awards, both from corporate compliance standards, and from the industry, such as the Bronze Award through the National Quality Award Program with the American Healthcare association. His passion is to improve the lives of the residents, families and staff he serves.
